Prior to participating on any of Adventure West Virginia Resort, LLC, dba Class VI - Mountain River's adventure trips, you will be required to read, and sign our forms. To access the form prior to your trip, we invite you to review and download our form.We require a separate form to be filled out for each activity our guests participate in. Occasionally groups will add an additional activity or change the activities once they have arrived. If your child will not be accompanied by their parent or legal guardian, we recommend downloading, signing and sending a form each activity with your child or group leader.(Minors are required to bring a signed form if their parent or legal guardian will not be present the day of the activity)
